The Kentucky Department of Education mandates every student have a current Commonwealth of Kentucky Immunization Certificate to attend school.

Here’s what your child’s immunization record must reflect:
  • All students must have two doses of Hepatitis A vaccine along with the dates your child received the two doses of Hepatitis A. The two doses of Hepatitis A vaccine must be given 6 months apart.  If both doses of Hepatitis A can't be administered before the start of the 2018-19 school year, your healthcare provider will give a provisional immunization certificate that will expire the time the next shot is due. Once your child has the second dose of Hepatitis A vaccination a new certificate will be given that you can submit to the School Nurse.
  • Students going entering kindergarten, must have a physical exam, a dental exam, an eye exam, and completed age appropriate immunizations. See the links below for immunization schedules.
  • Students going into sixth grade must have their immunizations updated and a physical exam (the form required by the state is different than the one required for sports physicals).
  • All students ages 16 years or older are also required to show proof of having two doses of the Meningococcal ACWY vaccine. If the first dose of Meningococcal ACWY was received at age 16 or older, the second dose is not required for school entry.

All students must submit an updated immunization record for the 2018-19 school year. Please send your child’s Certificate of Immunization to school as soon as possible. Certificates may be dropped off at your child’s school, emailed, faxed, or delivered to the School Nurse.
